BSA Annual Heath and Medical Record (AHMR)

BSA AHMR part A and B is required for all scouting events less than 72 hours. Events longer than 72 hours also need part C physical by health care provider. BSA AHMR forms and frequently asked questions here.

Activity Consent Form

Permission slip and consent for Scouting Activities

You may also print from scoutbook - go to events, select event you plan to attend, RSVP, select permission slips. This will print with the event and your scouts information auto filled. Just print and sign.

RSVP and permissions slips are typically due at least 2 weeks prior to events.

Scout Strong

To carry out all the duties and work of a Scout properly, one has to be strong, healthy, and active. Three programs PALA, Medwise and Healthy Unit Award. Program details at this link.
